29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Glacier Signs & Monuments, Kalispell MT: On or about October 27, 2021, and at times prior, employees were exposed to respiratory hazards while using a 3M Half Mask Elastomeric respirator to perform sand blasting and CNC machining operations to create headstones. The employer failed to provide medical evaluations to employees required to use the respirators. b) Glacier Signs & Monuments, Kalispell MT: On or about October 27, 2021, and at times prior, employees were exposed to respiratory hazards while using a 3M Half Mask Elastomeric respirator to perform sand blasting and CNC machining operations to create headstones. The employer failed to provide fit testing procedures for tight-fitting respirators to employees required to use the respirators. c) Glacier Signs & Monuments, Kalispell MT: On or about October 27, 2021, and at times prior, employees were exposed to respiratory hazards while using a 3M Half Mask Elastomeric respirator to perform sand blasting and CNC machining operations to create headstones. The employer failed to provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators.

The employer failed to ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.
