Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) 2424 Lincoln Hwy, Langhorne, PA, Roof: On or about October 28, 2021, employees were performing commercial roofing work, not less than 20 feet above grade, without utilizing any means of fall protection.

29 CFR 1926.502(d)(15)(i):29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: (a) 2424 Lincoln Hwy, Langhorne, PA, Roof: On or about October 28, 2021, employees were performing commercial roofing work and exposed to a fall hazard not less than 20 feet above grade, however, the complete personal fall arrest system in use by employees did not maintain a safety factor of at least two.
