29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about November 2, 2021, the employer did not ensure periodic inspections of energy control procedures were conducted annually, including a review of each authorized employee's responsibilities under the energy control procedures.  Authorized employees are required to conduct and direct service/maintenance activities on the Imabe Baler requiring them to perform lockout/tagout.

29 CFR  1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  On or about November 2, 2021, the employer had compressed air in the Baler and Side Dock areas that was not reduced to less than 30 psi. The compressed air measured at 50 psi at the Side Dock, 90 psi at the Baler, and was being used for cleaning.

29 CFR  1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  On or about November 2, 2021, a door in the Skid Room that could be mistaken as an exit was not marked appropriately potentially causing delays and/or harm to the employees attempting to exit the building during an emergency.
