29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  New residential construction located on Lot #58 at 2207 Hastings Manor Lane, Lovejoy, GA 30250, Roof: On or about 11/02/2021, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not provided to employees engaged in shingle installation activities operating these tools at 110 PSI and firing 1-1/4 metal collated coil roofing nails.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    New residential construction located on Lot #58 at 2207 Hastings Manor Lane, Lovejoy, GA 30250, Roof: On or about 11/02/2021, the employer exposed employees to fall hazards of approximately 10-feet, 1-inch from the lower ground level, in that a fall protection system was not provided to employees engaged in roofing activities on a 6/12 pitched roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    New residential construction located on Lot #58 at 2207 Hastings Manor Lane, Lovejoy, GA 30250, Front side of the structure: On or about 11/02/2021, the employer exposed employees to fall hazards of approximately 10-feet, 1-inch from the lower ground level, in that the ladder provided did not extend at least 3-feet past the roof landing to allow for safe access on and off the roof.
