29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal arrest system: Location: 261 E 202nd St. Bronx NY 10458 a) On or about November 3rd 2021, employees were exposed to a fall hazard while using a ladder as access to 2nd floor that was outside of the Control Access Zone, approximately 14ft to the ground.

29 CFR 1926.502(f)(2)(i):The rope, wire, or chain was not flagged at no more than 6-foot intervals with high-visibility material: Location: 261 E 202nd St. Bronx NY 10458 a) On or about November 3rd 2021, several employees were exposed to a fall hazard, approximately 14ft from the 1st floor to the ground, due not having any flags on the Warning Line System in place. b) On or about November 3rd 2021, several employees were exposed to a fall hazard, approximately 28ft from the 2nd floor to the ground, due not having any flags on the Warning Line System in place. c) On or about November 3rd 2021, several employees were exposed to a fall hazard, approximately 42ft from the 3rd floor to the ground, due not having any flags on the Warning Line System in place.
