29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  La Jolla Indian Campground, Zip Line Retrieval Platform 1: On and before October 30, 2021, employees were engaged in the recovery of clients near two unprotected edges that were 10 feet 4 inches and 7 feet above the ground below with no fall protection.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  La Jolla Indian Campground Zip Line: On and before October 30, 2021 employees rode in the back of a pickup truck standing to the rear of the truck bed leaning against a barrier chain where protective equipment was not installed and/or used such as seat belts. Employees were exposed to ejection hazards.

A hazard assessment was not conducted to determine the hazards present and the necessary personal protective equipment (PPE) for each hazard. Employee(s) were exposed to ejection and/or fall hazard.  b) La Jolla Indian Campground, Zip Line Retrieval and Launching Platforms/Areas: On and before October 30, 2021, Employee(s) worked on platforms/areas recovering and launching customers.  A hazard assessment and equipment selection was not conducted to determine the hazards present in the workplace and the necessary personal protective equipment (PPE) for each hazard.  Employees were exposed to fall and struck by hazards.
