Russia, OH: The employer did not ensure employees who operate the Clark model# C500-45 serial# 40-103-2520 forklift were adequately trained and authorized to operate such equipment. Inadequate training exposed operator and other employees working in and around the forklift to struck-by hazards.

Russia, OH: The employer failed to ensure adequate machine guarding was in place on LUX-MATTER Turn-Pro Lathe. Employees operating the machine in this condition were exposed to caught in hazards.
