On or about November 02, 2021, an employee was exposed to a fall hazard while standing on the midrail of the scissor lift basket.

29 CFR 1926.451(g)(4)(iii):When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform. On or about November 2, 2021 and times prior thereto, employee was exposed to fall hazard while operating a scissor lift (JLG 2632ES) without re-attaching the gate chain that functions as a midrail.
