Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about November 3, 2021, at the work site located at 865 Lombardi Avenue, Green Bay, Wisconsin, employee(s) were exposed to fall hazards while performing tear off and re-roofing activities on steep slope roof surfaces of a commercial property. Employees were exposed to an approximate 25 foot fall hazard without the use of conventional fall protection.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall: (iii) be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level: On or about November 3, 2021, at the work site located at 865 Lombardi Avenue, Green Bay, Wisconsin, employees were exposed to fall hazards while installing new shingles on a steep slope roof surface of a commercial property. Two employees were wearing body harnesses with a rope grab system. The connecting device for the two employees was rigged at the end of the rope-grab lifeline and would have allowed the employees to fall over the eave and fall more than six feet.
