Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. Location - 143 Forest Street, Arlington, MA. On or about November 3, 2021, employees were not protected from falling while working/walking on a roof greater than twelve feet above lower level.
