29 CFR 1926.1412(d)(1): A visual inspection for apparent deficiencies of the equipment used was not conducted by a competent person prior to each shift the equipment was used or was not completed before or during that shift: On or about June 6, 2021 and there after, employer did not produce copies of daily, weekly, and monthly crane inspections. This condition exposed employees to struck-by hazards.
