1926.454(c)(3) The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: On or about November 3, 2021, at a worksite located at or about 1365 North Westfield Street in Oshkosh, Wisconsin; while installing light ballasts on the porte cochere of a new hotel construction, two employees were exposed to fall hazards and crushed-by hazards while standing on the midrail and extending off the side of a scissor lift and operating the lift with a defective switch.
