OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employees were exposed to fall hazards while working from an elevated work platform: Warehouse: Employees worked in a job-made box, placed on a Crown Stand-Up Rider forklift, while dismantling a racking systems at heights over 10 feet from the concrete floor. The job-made box, used as a personnel platform, was not secured to the lifting carriage and did not have standard guardrails along all sides. An unapproved anchorage point was used as protection from falling.
