29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a construction site located at 8511 N McKinley Ave, Kansas City, Missouri: Two employees were exposed to fall hazards. This was most recently documented on or about October 20, 2021 when two painters were observed utilizing caulk on the roof of a house. The painters were exposed to a fall of greater than 13 feet without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Worksite located at 8511 N McKinley Ave., Kansas City, Missouri: Employees were exposed to fall hazards while gaining access to the roof. This was most recently documented on or about October 20, 2021 when the ladder side rails for two ladders did not extend at least 3 feet above the upper landing surface being accessed. One ladder was used to access the ground to the lower roof. The second ladder was used to access the lower roof to the next higher section of roof.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): Worksite located at 8511 N McKinley Ave., Kansas City, Missouri: Employees were exposed to hazardous chemicals and there was no written hazard communication program. This was most recently demonstrated on or about October 20, 2021 when employees were using Powerhouse 1100A Siliconized Acrylic Latex Sealant and Top Gun 400 Elastomeric Urethane Sealant. Powerhouse 1100A Siliconized Acrylic Latex Sealant can cause skin corrosion, skin irritation, and eye damage. Top Gun 400 Elastomeric Urethane Sealant can cause respiratory tract and eye irritation.
