On or about November 02, 2021, on the east side of the commercial construction site located on 21721 Merchants Way in Katy, TX, employees were exposed to a fall hazard while working from a pump jack scaffold that did not have an adequate foundation on base plates and mud sills.

29 CFR 1926.452(j)(2):Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. On or about November 02, 2021, on the east side of the commercial construction site located on 21721 Merchants Way in Katy, TX, two employees were exposed to a fall and struck-by hazards when working on an elevated pump jack scaffold without the scaffold braces at the necessary points.
