29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Facility - On or about June 23, 2021, an employee operating a press brake (Press Brake #8: Accurpress, Model #412012, Serial #11925) bending 18 inch steel stakes experienced a work-related amputation to his right 4 fingers. The employer reported this to OSHA on November 2, 2021.
