On or about November 4, 2021, west side of the project site, an employee was straddling the top rail of a Genie Scissor lift while performing work on the awning of a building at heights upwards of ten (10) feet.

On or about November 4, 2021, employees on the site's westside were performing awning/canopy work at heights above six feet without adequate fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about November 4, 2021, employees on the west side of the site performed canopy work, using a Louisville twenty-foot extension ladder to access the upper level of the canopy, which was not extended three feet above the upper landing.
