The Building 21 K-18 location is in the area covered by 29 CFR 1910.119 Process Safety Management of Highly Hazardous Chemicals.  b) On or about October 1, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the Building 21 K-18 location, the signed Hot Work Permit did not specify the object(s) that were to be welded and did not ensure the proper location was determined safe from fire hazards. The Hot Work Permit description of Work Being Performed was "welding" and did not identify the object on which the hot work was to be performed on. He shall designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit:  a) On or about September 21, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the BMC Tank Farm Area, the signed Hot Work Permit did not indicate the time period the welding of decking in the mezzanine area could safely be performed. The signed Hot Work Permit Started Time and Permit Expires Time were not documented.   b) On or about September 21, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the Warehouse Dock Area, the signed Hot Work Permit did not specify the object(s) that were to have welding and grinding accomplished, and did not ensure the proper location was determined safe from fire hazards. The signed Hot Work Permit Description of Work Being Performed was "grinding, welding".  c) On or about September 21, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the BMC T-206 Area, the Hot Work Permit was not authorized by a Permit Authorized Individual (PAI). The Hot Work Permit was not signed by the Permit Authorized Individual (PAI). Also, the Hot Work Permit did not specify the object(s) being welded and did not ensure the proper location was determined safe from fire hazards. The Hot Work Permit Description of Work Being Performed was "Welding".   d) On or about September 24, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the BMC Tank Farm Area,  the signed Hot Work Permit did not specify the object(s) being welded and did not ensure the proper location was determined safe from fire hazards. The signed Hot Work Permit Description of Work Being Performed was "Welding".     e) On or about October 20, 2021, at an establishment located at 1720 North Avenue, Sheboygan, Wisconsin, in the BMC First Floor Area, the signed Hot Work Permit did not indicate flammable or combustible materials had been removed from the immediate area and that no atmospheric hazard was present. The Superjet-S-Eutalloy Case Hardening Torch had been repaired with parts from another non-operational Superjet-S-Eutalloy Case Hardening Torch instead of performing the manufacturer's recommended maintaining, servicing and fault-finding procedures.
