Authorized employees perform servicing and maintenance activities on the wire bender machine requiring energy control. Employees were exposed to electrical, pneumatic, hydraulic energy sources.

Employees engage in the service and maintenance activities on machinery and equipment including, but not limited to; wire bender and other machinery. Employees were exposed to electrical, pneumatic, and hydraulic energy sources.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about November 4, 2021, the employer did not ensure one or more methods of machine guarding were provided to protect the operator and other employees working at the Everising automatic band saw during production from the ingoing nip points created by the chip auger. The access door/cover was removed and not in place during the saw operation. b. On or about November 4, 2021, the employer did not ensure one or more methods of machine guarding were provided to protect the operator and other employees from the rotating blade on the Ellis band saw. The idler wheel and drive wheel housing access doors/guards would not stay closed due to the magnets no longer keeping the doors closed. The idler wheel access door was open while the saw was cutting.
