29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about November 1, 2021 and times prior there to, the employer did not ensure employees exposed to flying debris were provided eye protection in the following instances:  a) Two employees on the roof working with pneumatic nail guns; b) Two employees on the ground working with electric saw.

On or about November 1, 2021 and times prior there to, the employer did not ensure that employees actively performing residential construction duties more than six feet above the lower level were protected with a personal fall protection system. 29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables were used.  On or about November 1, 2021 and times prior there to, the employer did not ensure frayed electrical cords were not being used to power electrical equipment being utilized by employees on site.
