29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips.  (a)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer failed to ensure the Enco horizontal lathe used in the maintenance shop area had an effective guard in place to protect employees from the rotating work holding device, or chuck.  This hazard exposes employees to crushing, caught in between and amputation hazards.  Abatement Note:  The National Consensus Standard, ANSI B11.6-1975, Safety Requirements for the Construction, Care, and Use of Lathes. Section 5 of this standard contains the safeguarding requirements. Paragraph 5.3 states:  Work-Holding-Device Hazard. When the work holding or driving device in the clamped mode has components that extend beyond the outside diameter of the holding device; or the periphery of the body of the holding device is of irregular shape; or a pinch point is created and the operator is not effectively protected by location, machine components, or other means from coming into contact with these projections; a fixed or moveable guard, device, awareness barrier, or peripheral cover over area exposed to the operator shall be required.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one- fourth inch:  (a)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer did not ensure that the tongue guard on a bench grinder was adjusted when necessary.  The distances between the abrasive wheel periphery and the adjustable tongue exceeded 1/4 inch on both the right and left side of  Dayton 10-inch bench grinder located in the maintenance area.  Additionally, the left abrasive wheel was coated with a metallic substance that could be shed and become airborne when the wheel is activated.   This condition exposed employees to flying debris hazards associated with a abrasive wheel failure as well as metal debris leaving the wheel while in use.

29 CFR  1910.242(b):Compressed air used for cleaning purposes was not reduced to less than 30 pounds per square inch  (p.s.i):  (a)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer did not ensure that compressed air nozzles used for cleaning were regulated so as not to exceed 30 p.s.i.  Employees regularly use compressed air to clean off working surfaces and general maintenance cleaning in the compressor room.   This condition exposes employees to embolism and eye injuries from flying debris.

29 CFR  1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  (a)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer failed to ensure all chemical containers were properly labeled.  A 55-gallon drum with a pump handle, full of calcium chloride was being used in the production building and did not have a label identifying its contents, exposing employees to chemical hazards.  (b)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer failed to ensure all chemical containers were properly labeled.  A spray bottle with an unknown liquid was present on a shelf on the east side of the main building production floor.  This condition exposes employees to unknown chemical hazards if mishandled.

29 CFR  1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical:  (a)Raven Industries, Inc. Engineered Films Division at 500 SE 12th Street, Madison, SD 57042:  On or about and at times prior to November 4, 2021, the employer did not ensure chemical Safety Data Sheets (SDS) were available for all chemicals that employees were exposed to in the workplace.  These chemicals include, but are not limited to, Kimball Midwest Ultra-cut cutting tool coolant 80-801 and FIC ultra heavy duty penetrating oil.  This condition exposed maintenance and other employees to hazards associated with chemical exposure.
