29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Production Work Area: On or about November 3, 2021, employees operated machinery, including a Heller Plate Bending Machine that was not guarded at the point of operation, resulting in an employee's hand being caught between two in-running rollers and the amputation of two fingertips.
