29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Location: Wood Shop On 10/20/2021, an employee was struck by a table saw blade while while cutting wood in the D.E.L.S. Inc. woodshop and he was subsequently admitted into the hospital for surgical repairs. D.E.L.S. Inc. was required to report the incident to OSHA within 24 hours; the incident was reported to OSHA by the Massachusetts Department of Public Health on 11/05/2021
