29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings, where the outside bottom edge of the wall opening was 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: (a) On or about November 5, 2021, on the west side of the second floor, where an employee was exposed to a fall hazard greater than 6 feet while installing sheathing boards near a wall opening without fastening the body harness or using an adequate horizontal lifeline.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about November 5, 2021, on the west side of the second floor, where an employee was exposed to a free fall hazard greater than 6 feet while installing sheathing boards near a wall opening when using a horizontal lifeline that did not prevent contact with the surface below.
