29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a) On or about November 3, 2021, at the residence where employees were exposed to fall hazards exceeding six feet when replacing doors at an elevator shaft without a fall protection system.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) On or about November 3, 2021, on the second floor, where employees tasked with replacing doors were exposed to a fall hazard when the second floor landing railing was not equipped with a midrail or equivalent.
