29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about 10/12/2021, CPA Roofing Consultants LLC and their subcontractor were installing roofing materials on a roof of a commercial building located at 1203 East Veterans Memorial Parkway in Warrenton, MO. CPA Roofing Consultants LLC, the controlling employer, did not conduct an adequate inspection of the job site, materials, and equipment. Subcontractor employees were exposed to the following fall hazards: a) The subcontractor was installing roofing materials on a 3/12 sloped roof, approximately seventeen feet above the ground, without fall protection. Three employees were not using guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. b) Three subcontractor employees were using an extension ladder to access the roof. The ladder was not extended three feet beyond the landing or edge of the roof.
