OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Location: 69 Blanchard St Newark, NJ 07105 - Manufacturing   a)Employees engaged in warehousing activities are exposed to crushing hazards when operating forklifts without using the provided seat belt, or other means of restraint, to ensure they remain safely inside the operator's compartment in the event of sudden unexpected motion. The employer did not ensure that forklift operator restraint devices or seat belts were used. Violation observed during the time of the inspection walkthrough on or about 11/8/21.

29 CFR  1910.178(n)(4):The driver was not required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.   Location: 69 Blanchard St Newark, NJ 07105 - Manufacturing   a) Employees exposed to struck by hazards while forklift and electrical pallet jack operators drove  in close proximity to other drivers and walking pedestrians, without signaling in and out of traveling lanes and in areas where views were obstructed by materials on or about 11/8/21.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Location: 69 Blanchard St Newark, NJ 07105 - Manufacturing   a) Employee exposed to amputation hazards while conducting an  instructional demonstration and reaching into a live SCM Olimpic k 560 automatic edge bander machine compartment with ingoing nip points and rotating blades. The incident occurred on or about 11/2/21.
