29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Site - An employee was not protected from falls of approximately 12 feet when installing foam board insulation on an exterior wall while working from a pitched roof without the use of fall protection. Manuel Elias Mayancella DBA- Spring Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1522563, citation number 001, item number 001, and was affirmed as a final order on May 7, 2021, with respect to a workplace located at 7 Kilton Rd. Bedford, NH 01757.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Site- An employee using an aluminum extension ladder to gain access to and from the was exposed to falls of approximately 12 feet when the side rails of the ladder did not extend past the roofs eave more than a couple of inches.
