29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. At 1900 Copperstone Drive, Lot's A-F, Fleming Island, Florida: On or about November 9, 2021, the employer exposed employees to head injuries, in that protective helmets (hard hats) were not utilized by employees who worked beneath other employees, while performing roofing operations.

At 1900 Copperstone Drive, Lot's A-F, Fleming Island, Florida: On or about November 9, 2021, the employer exposed employees to eye injuries caused by air-born debris and flying objects, in that eye protection was not utilized, while a gas-powered  blower was operated during the removal of an existing roof system.

At 1900 Copperstone Drive, Lot's A and E, Fleming Island, Florida: On or about November 9, 2021, the employer exposed employees to a 10-foot fall hazard, in that a 28-foot, aluminum extension ladder, consisting of two sections, was separated into two individual ladders and the flying section of the ladder, which did not have slip-resistant feet, was as used to access and egress the roof, which was a purpose other than the purpose for which the ladder was designed.
