29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards.  The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  On 11/8/2021, employees of Gonzalez Roofing, LLC were performing roofing work on a residential building located at 512 Meadowfield Court, Wentzville, Missouri.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.  The employees were observed on the roof using ropes without anchor points and equipment not approved for construction use.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record:  On 11/8/2021, employees of Gonzalez Roofing, LLC were performing roofing work on a residential building located at 512 Meadowfield Court, Wentzville, Missouri.  The employer did not prepare a written certification verifying that the workers exposed to fall hazards in the course of their work had been properly trained.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 11/8/2021, employees of Gonzalez Roofing, LLC were performing roofing work on a residential building located at 512 Meadowfield Court, Wentzville, Missouri.  A multi-position and an extendable Ladder were positioned on the residence to access and egress the roof.  The side rails of the ladders did not extended 3 feet above the upper surface, exposing employees to fall hazards.

29 CFR 1926.501(b)(13):  "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 11/8/2021, employees of Gonzalez Roofing, LLC were performing roofing work on a residential building located at 512 Meadowfield Court, Wentzville, Missouri.  Employees had removed the old shingles and starting to install the roofing felt on a roof.  The pitch of the roof was 8:12 and 10:12 with a height of approximately 11-18 feet above a lower level.  Employees were not utilizing approved guardrail systems, a safety net system, or a personal fall arrest system.    Gonzalez Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1394807, citation number 1, item number 2 and was affirmed as a final order on 10/14/2019, with respect to a workplace located at 82 Kaestner Drive, Dupo, IL.
