29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  On or about November 9, 2021, employees of Port City Bakery dba Alive & Kickin' Pizza Crust were exposed to machine hazards when: a) working with or near the Bowl Lift on Line #6  which had not been guarded to prevent access to moving parts and the lifted lug of dough and exposed them to struck-by and caught-in hazards. b) removing excess dough from the Line #6 Kaak Group Inc. Allround Rounders where the lower guide and cone create an entanglement point.  c) working with or near the Bowl Lift on Line #5  which had not been guarded to prevent access to moving parts and the lifted lug of dough and exposed them to struck-by and caught-in hazards. d) working with or near the Bowl Lift on Line #4  which had not been guarded to prevent access to moving parts and the lifted lug of dough and exposed them to struck-by and caught-in hazards.

29 CFR  1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving.   On or about November 9, 2021, employees were exposed to machine hazards associated with unsecured machinery when a Jet Model J-2550 Drill Press in the maintenance shop was not anchored to the floor.

29 CFR  1910.215(b)(9):Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  On or about November 9, 2022, the tongue guard for the Dayton Model 2LKR6B Bench Grinder in the maintenance shop was not adjusted to within one quarter of an inch as required by the standard and exposed employees to potential struck by hazards in the event of a stone fracture.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Employees of Port City Bakery dba Alive & Kickin' Pizza Crust were exposed to machine hazards associated with the point of operation: a) on or about November 2, 2021, when the Line #4 Divider was not guarded to prevent access through the end tunnels of the Divider. An employee reached into the end of the Divider to remove excess dough that was leaking onto the conveyor below and their finger was caught in a self-closing aperture resulting in an amputation. b) on or about November 9, 2021, when the Line #5 Divider was not guarded to prevent access through the end tunnels of the Divider and exposed employees to potential caught-in hazards.

29 CFR  1910.219(c)(4)(i):Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.  On or about November 9, 2021, employees working in the Line #6 Packaging Department were exposed to potential entanglement hazards when a rotating shaft at the cardboard filler station had a set screw protruding from the shaft.

29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about November 9, 2021, Port City Bakery dba Alive & Kickin' Pizza Crust had not conducted a Periodic Inspection of the Line #4 Divider procedure to include a demonstration of the procedure being evaluated.
