Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews: On 11/4/2021, employees of Clark Glaser Masonry, Inc. were performing masonry work on a residential building located at 1895 North Service Road East, Warrenton, Missouri. Employees were working from a fabricated frame scaffold approximately 11 feet above the lower level without installing a guardrail system along all open sides and ends.

29 CFR 1926.602(d): The employer did not train powered industrial truck drivers as required by paragraph 29 CFR 1910.178(l): On 11/4/2021, an employee of Clark Glaser Masonry, Inc. were performing brick laying operations on a residential building located at 1895 North Service Road East, Warrenton, Missouri. The employee was operating a SkyTrak 6036 to lift pallets of brick to the elevated portion of the scaffold. The employer had not ensured that the operator had received powered industrial truck training as required by 29 CFR 1910.178(l).
