29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Location - 140 Pond Road, Billerica, MA. 01821. On or about November 11, 2021, employees were exposed to struck-by and crushed-by hazards when an employee was allowed to operate a forklift without the use of an approved lifting attachment, but rather via utilization of "free rigging" (direct attachment to or placement of rigging equipment; slings, shackles, rings, spreader bars etc.) onto the tines of a powered industrial truck for a below-the-tines lift.
