29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the jobsite, employees were exposed to fall hazards of approximately 8-feet 5-inches and greater when they used an aluminum extension ladder at the south side of the house without extending the ladder three feet above the landing.   b) At the jobsite, employees were exposed to fall hazards of approximately 8-feet 5-inches and greater when they used an aluminum extension ladder at the northwest corner of the house without extending the ladder three feet above the landing.   c) At the jobsite, employees were exposed to fall hazards of approximately 8-feet 5-inches and greater when they used an aluminum extension ladder at the entranceway of the house without extending the ladder three feet above the landing.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a) At the jobsite, employees were exposed to fall hazards of approximately 8-feet 5-inches and greater when they used an aluminum extension ladder at the south side of the house with an unsafe angle.  The horizontal distance from the top support to the foot of the ladder was  approximately 5-feet 2-inches; the distance was required to be 2-feet 2-inches at this working height (one-quarter of the working length of the ladder).   b) At the jobsite, employees were exposed to fall hazards of approximately 8-feet 5-inches and greater when they used an aluminum extension ladder at the south side of the house with an unsafe angle.  The horizontal distance from the top support to the foot of the ladder was  approximately 5-feet 4-inches; the distance was required to be 2-feet 2-inches at this working height (one-quarter of the working length of the ladder).
