29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about November 10, 2021 an employee at the 244 Main Street, Farmington, CT construction project was exposed to falls while engaged in framing activities without fall protection in the following locations:  A)  An employee was exposed to approximately 10' falls through an unprotected hole opening in the center of the newly constructed floor deck.  B) An employee was exposed to approximately 10' falls through an unprotected hole opening of  a current stairwell opening.
