A job-made hoisting system was used, however, was not approved for personnel hoisting by a registered professional engineer, in writing, prior to use: a) 92 Fort Salonga Rd., Northport NY - Rear yard: Employee entered a drywell to remove silt and mud from the bottom. Employee was lowered into the drywell riding on a chain attached to the bucket of an excavator (Link Belt 160LX serial K2J5-9519); on or about 11/9/2021. The hoist system was not designed by a professional engineer. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.
