Lewis Ave. in Tulsa, OK, employees were observed working on a scaffold that did not have a safe means of access, exposing the employee to the hazard of falls.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level:                                                                                                                                          On or about November 9, 2021, at a new apartment complex construction site located at 62 N. Lewis Ave. in Tulsa, OK, employees were observed working on a scaffold that did not have guardrails around all open ends, exposing the employees to the hazard of falls.
