On or about November 12, 2021, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2021:  (a) On or about June 8, 2021, an employee performing electrical work sustained head injuries requiring hospitalization. The employer did not record this work-related injury on the OSHA Form 300.

The employer did not report to OSHA the hospitalization of an employee within 24 hours.
