On or about November 10, 2021, at the above addressed jobsite, an employee working on the ground, below siding installation activities, was not wearing a hard hat to protect against falling materials and debris.

On or about November 10, 2021, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  b.  On or about November 10, 2021, at the above addressed jobsite, an employee using a chop saw did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about November 10, 2021, at the above addressed jobsite, an employee conducting siding operations from a ladder jack scaffold, approximately thirteen (13) above the ground, was not provided with an access ladder, thereby exposing the employee to falls.
