Located at 7624 Haverington Street in Pickerington, Ohio: On or about November 12, 2021, employees were climbing from the top step of a six foot step ladder to access the top of the first story walls. The top of the ladder did not extend at least three feet above the landing, thereby exposing employees to a fall hazard. b. Located at 7624 Haverington Street in Pickerington, Ohio: On or about November 12, 2021, employees were using an aluminum extension ladder to access the second floor. The top of the ladder did not extend at least three feet above the landing, thereby exposing employees to a fall hazard.
