29 CFR  1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  (a) On or about  November 9, 2021, the employer failed to identify and evaluate respiratory hazards in the produce prep room at the  workplace, so as to determine effective means of respiratory protection for employees performing cleaning operations.  Employees working in the produce prep room were exposed to hazardous levels of carbon monoxide resulting from operating a gasoline-powered pressure washer inside a room with no windows or fresh air ventilation provided.
