29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles. On or about November 12, 2021, and at times prior thereto, one employee was using a pneumatic nail gun to secure shingles to the roof of a new home under construction without wearing eye protection, exposing the employee to the hazard of being struck-by flying debris or projectiles.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about November 12, 2021, and at times prior thereto, an employee was securing shingles to the roof of a residential home under construction up to approximately 30 feet high without fall protection, exposing the employee to a fall hazard.
