29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Woodsboro Apartments, Building 8576-8586, Baldwinsville, NY  13027, on or about 11/10/21:  Workers were accessing a 6:12 pitch roof with an extension ladder that was placed against the rake of the roof, allowing for 2 feet 9 inches above the upper landing on the downslope side of the ladder, and 1 foot 6 inches on the upslope side of the ladder.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Woodsboro Apartments, Building 8576-8586, Baldwinsville, NY  13027, on or about 11/10/21:  Workers were installing roofing material onto a 6:12 pitch roof with ground to eave heights of 9 and 18 feet without fall protection of any kind.
