29 CFR  1910.22(a)(1):(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) On or about November 16, 2021 in the storage room between the chain sharpening room and the public store, walking and working surfaces were not clean and orderly in that containers, boxes, tools, and equipment were stored on the floor exposing employees to tripping hazards.  b) On or about November 16, 2021 in the chain sharpening room, walking and working surfaces were not clean and orderly in that containers, boxes, tools, and equipment were stored on the floor exposing employees to tripping hazards.  c) On or about November 16, 2021 in the engine repair room, walking and working surfaces were not clean and orderly in that containers, boxes, tools, and equipment were stored on the floor exposing employees to tripping hazards.  d) On or about November 16, 2021, the steps in the storage room were missing the center 1 x 3 inch section in the front center of the middle step exposing employees to a tripping hazard.

29 CFR  1910.25(b)(3): Stairs have uniform riser heights and tread depths between landings.  a) On or about November 16, 2021 in the chain room, employees are exposed to a fall and trip hazard due the height of the steps being different between the three steps. The height of the first step was 3 inches and the height of the two remaining steps were 6.5 inches.  b) On or about November 16, 2021 in the chain room, employees are exposed to a fall and trip hazard due to the depth of the tread being different for the three steps. The center step was missing a 1 inch by 3 inch section of tread, and a wood fiber board covered pallet was the third step.

1910.38(c) Minimum elements of an emergency action plan. An emergency action plan must include at a minimum: (1) Procedures for reporting a fire or other emergency; (2) Procedures for emergency evacuation, including type of evacuation and exit route assignments; (3) Procedures to be followed by employees who remain to operate critical plant operations before they evacuate; (4) Procedures to account for all employees after evacuation; (6) The name or job title of every employee who may be contacted by employees who need more information about the plan or an explanation of their duties under the plan.  a) On or about November 16, 2021, the employer, who would evacuate in case of an emergency, had not completed an emergency action plan that addressed reporting a fire, evacuation procedures, exit routes, and who to contact for additional information for the hardware store that included a gas station, a propane fueling station, and small engine repair shop on gas powered engines.

29 CFR  1910.176(c):Housekeeping. Storage areas shall be kept free from accumulation of materials that constitute hazards from tripping, fire, explosion, or pest harborage. Vegetation control will be exercised when necessary.  a) On or about October 25, 2021 in the fenced exterior storage area and around the exterior of the store, the employer exposed employees to tripping hazards from vehicle ruts, a runoff drainage ditch, broken pallets, hidden piping, and other debris when employees were getting pipes for customers.  b) On or about November 16, 2021 in the fenced exterior storage area and around the exterior of the store, employees were exposed to pest harborage such as but not limited to raccoons that used the excessive brush to access the roof and enter into the store.  c) On or about November 16, 2021 in fenced exterior storage, the propane fuel station, and around the store, employees were exposed to fire hazards from the accumulated brush, combustible debris such as pallets, and from unsafe storage of  tanks that could contain residual amounts of LPG gas.
