29 CFR 1910.106(f)(8): Additional fire-control equipment was not provided as required for a tank of more than 50,000 gallons individual capacity containing Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 �F (37.8 �C), and where an unusual exposure hazard existed from surrounding property. Such additional fire-control equipment shall be sufficient to extinguish a fire in the largest tank. The design and amount of such equipment shall be in accordance with approved engineering standards: Tank Gallery underground tunnel: The AFFF fire suppression system for the 12-million gallon storage tanks which contained hydrocarbon fuels, was disabled and locked out for more than a year. Welding and hot work on the tanks, and the tank gallery area of the tunnel was ongoing. Alternative firefighting measures were not put in place, exposing employees to fire hazards.

29 CFR 1910.160(b)(2): The employer did not notify employees and take the necessary temporary precautions to assure their safety when the fixed extinguishing system became inoperable. Tank Gallery underground tunnel: The AFFF fire suppression system was disabled and locked out for more than a year. Alternative firefighting measure were not put in place and employees working in the tunnel were not notified of the condition of the AFFF system. Employees were exposed to fire hazards.
