29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves. On or about November 5, 2021, and at times prior thereto, the employer did not ensure that the projecting shaft end of the hammer mill was protected or guarded to prevent employees from being caught-in or struck-by the rotating shaft.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about November 5, 2021, the employer did not report to OSHA a work-related hospitalization prior to the elapse of the 24 hour reporting period after learning about the hospitalization. The incident was not reported to OSHA until November 8, 2021.
