29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. New townhome construction located on Lots #112 and #113 Catamaran Court, Alpharetta, GA 30009, Rear side: On or about 11/16/2021, the employer exposed employees to fall hazards of approximately 30-feet to the lower ground level, in that a fall protection system was not provided while employees were engaged in siding installation activities using a pump jack scaffold system.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section: New townhome construction located on Lots #112 and #113 Catamaran Court, Alpharetta, GA 30009, Rear side at cut bench: On or about 11/16/2021, the employer exposed employees to silica exposure while cutting Hardie Plank Siding, in that the DeWalt (Model DWE575) 7-1/4-inch circular saw used to cut siding was not provided with the vacuum system (Engineering Control) needed to reduce exposure of the silica dust created while cutting fiber cement siding.
