29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about, November 12, 2021, at or near 2441 15th Street North in Wahpeton, North Dakota, for the employees exposed to amputation and struck by hazards where the employer failed to ensure that specific Lock Out / Tag Out procedures were utilized, while performing maintenance activities on the Picker Bin Infeed system.
