29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: 238 Holyoke St., Ludlow, MA: On or about November 10, 2021, an employee was observed utilizing a fold out ridge anchor as part of a fall protection system that had 4 common nails installed in it and not the required lag screws as required by the manufacturer in order to obtain the strength needed to support 5000 lbs. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
