29 CFR 1910.28(b)(2)(i):  The employer did not ensure: (i) Each employee in a hoist area was protected from falling 4 feet (1.2 m) or more to a lower level by: (A) A guardrail system, (B) A personal fall arrest system or (C) A travel restraint system:   (a)  On or about November 16, 2021 , and at times prior, employees were not protected from falling greater than ten feet to a lower level while unloading tires from the conveyor onto the second story platform without a fall protection system in place.
